Transforming Care for Non-Communicable Diseases and Injuries (NCDIs) The NCDI Phase 3 Project builds on earlier efforts to strengthen Nigeria’s response to non-communicable diseases and injuries—conditions that continue to impose a heavy burden on individuals, families, and the health system. The project focuses on integrating NCDI services into primary healthcare, improving access to essential diagnostics and treatment, and promoting equitable, data-driven health planning.
Our Approach
HSDF adopts a systems-strengthening approach that ensures NCDI prevention and management are not treated in isolation but as integral to overall health system performance. By combining capacity building, policy support, and community engagement, the project drives sustainable change across multiple levels of care.
We collaborate with state and national health authorities to embed NCDI interventions into existing health strategies, ensuring alignment with broader public health goals. This integration promotes ownership, sustainability, and efficient resource utilization.
Through targeted training and mentorship programs, we equip healthcare providers with the skills to diagnose, treat, and manage NCDIs effectively within PHC settings. This empowers frontline workers to deliver comprehensive, person-centered care.
Data-Driven
HSDF works with community structures to raise awareness of NCDIs and reduce stigma around chronic conditions. This engagement promotes early diagnosis, adherence to treatment, and community-level ownership of health outcomes.
