Primary healthcare facilities in the FCT continue to face significant challenges, including manpower shortages, weak handovers, poor teamwork, faulty tools, and referral defaults – all of which hinder effective diabetes and hypertension care.
With support from the World Diabetes Foundation, the Integrated Diabetes Awareness and Care (IDAC) Project is addressing these issues head-on. By mentoring health workers, supplying essential consumables and data tools, and collaborating with partners, IDAC is strengthening chronic disease management across 64 facilities in the FCT.
These interventions are already delivering positive results. Health workers report greater confidence in managing diabetes and hypertension, facilities are improving documentation and referrals, and patients now have more reliable access to screening and treatment. Communities are also seeing stronger and more consistent service delivery.
Through interactive mentorship, practical demonstrations, and the real-time provision of data tools, IDAC ensures that knowledge is effectively translated into practice, building sustainable systems and improving the quality of care.
