HSDF's work is anchored on five interconnected pillars that shape how we understand challenges, design solutions, implement programmes, strengthen systems, and sustain results.

Evidence Generation

Building the knowledge base that drives every decision, adaptation, and system change.

HSDF begins every engagement with evidence. We establish baselines, conduct evaluations, generate new knowledge, and continuously gather insights throughout implementation to ensure programmes are informed by data, context, and learning rather than assumptions.

Co-Creation with Stakeholders

Designing solutions from within—with governments, communities, and the private sector.

HSDF works alongside those who will ultimately own and sustain change. Every intervention is designed collaboratively with government institutions, development partners, frontline workers, communities, and private sector stakeholders.

Conceptual Design

HSDF helps partners understand root causes, identify opportunities, and design interventions grounded in evidence and local realities. By combining analytical rigour with contextual knowledge, we ensure programmes are built for real-world conditions from the outset.

Strategy Development

Turning ambition into actionable plans.
We support governments and partners to develop national health plans, PHC strategies, investment cases, operational roadmaps, and implementation frameworks that are practical, evidence-based, and designed for ownership and execution.

Programme Implementation

Delivering results alongside governments and partners.
HSDF serves as an embedded technical partner, supporting direct implementation, capacity building, quality improvement, health systems strengthening, and data systems deployment. We work from within existing structures to accelerate delivery and strengthen institutions.

Monitoring, Evaluation & Learning

Creating systems that learn and adapt.
Through performance management systems, accountability mechanisms, knowledge management, and adaptive learning approaches, HSDF helps programmes continuously assess progress, respond to emerging challenges, and improve performance over time.

Scale-up & Sustainability

Embedding change for long-term impact.
HSDF designs for sustainability from day one. We support transition planning, government ownership, institutional anchoring, and capacity strengthening to ensure reforms continue delivering value long after project funding ends.

Niger State PHC Data Improvement

Strengthened routine health information systems through automated analytics, data quality improvement tools, and government-led performance management.
Results: 92.5% reporting rate, 94% data validity, and 80%+ private sector reporting.

Project Aisha

Applied real-time quality improvement approaches to improve maternal health outcomes across facilities in Lagos and Kaduna.
Results: 50% reduction in maternal deaths and 61% reduction in major obstetric complication-related deaths.

Connect & Learn for Sustainable Associations - CLOSA

Strengthened private sector associations across Ghana, Nigeria, and Senegal through capacity building and sustainability support. Results: 66 associations strengthened and NACOTAN membership increased to over 200,000.

ANRiN Knowledge Management & Learning

Supported learning, evidence use, and knowledge sharing to improve nutrition programme implementation across Nigeria.
Results: Enhanced visibility of programme performance and strengthened decision-making through data and learning products.

Nigeria Quality Initiative

Improved quality of care through facility-level quality improvement approaches and performance monitoring systems. Results: Stronger service delivery processes and improved use of quality improvement methodologies.
