Diabetes Awareness and Care Project

The Diabetes and Care (DAC) Project is a pioneering initiative dedicated to improving the prevention, diagnosis, and management of diabetes in Nigeria. With diabetes emerging as a growing public health challenge, DAC was established to strengthen health systems, build provider capacity, and improve patient outcomes through innovation, research, and community-centered care.

Our Approach

DAC integrates evidence-based strategies with digital tools to address gaps in diabetes care. The project emphasizes:

Early detection is at the core of DACโ€™s strategy. Many people living with diabetes remain undiagnosed until complications set in. By expanding access to affordable and routine screening services across primary and secondary health facilities, the project ensures that individuals can be identified at the earliest stages of the disease. This proactive approach reduces the risk of long-term complications, lowers the burden on tertiary health systems, and gives patients a better chance at effective management from the onset.

A strong health system is critical to tackling diabetes. DAC invests in strengthening existing structures by equipping facilities with the necessary resources and building the capacity of healthcare workers to provide high-quality, standardized care. The project emphasizes sustainable improvementsโ€”ensuring that from diagnostic equipment to treatment protocols, every part of the health system is prepared to deliver consistent and reliable care for people living with diabetes.

Communities are at the heart of diabetes prevention and care. DAC works closely with community members to increase awareness about the disease, its risk factors, and preventive practices. Through sensitization campaigns, educational programs, and partnerships with local influencers, the project empowers individuals to take charge of their health. By making knowledge accessible and relatable, DAC helps people adopt healthier lifestyles and manage diabetes more effectively within their daily environments.

Data is the engine that drives informed decision-making. The DAC Project leverages technology through its interactive dashboard to capture, analyze, and share real-time insights on diabetes trends, care outcomes, and project progress. This evidence base not only guides program implementation but also supports policymakers, donors, and healthcare stakeholders in shaping effective responses to non-communicable diseases. By transforming raw data into actionable intelligence, DAC ensures accountability and impact.
