Strengthening Diabetes Care in Imo State through Coaching and Mentorship.

As part of the Integrated Diabetes Awareness and Care (DAC) Project, HSDF—working in collaboration with the Imo State Ministry of Health and SPHCDA—is delivering targeted coaching and mentorship across 78 primary healthcare centres in 12 LGAs. Supported by the World Diabetes Foundation, this initiative builds frontline capacity for diabetes screening, diagnosis, referral, and follow-up care.

Despite infrastructural limitations, the commitment of health workers and the adoption of innovative practices such as Diabetes Corners, Clinic Days, group counselling, and leveraging immunization clinics to reach men—are driving meaningful impact. These approaches not only enhance service delivery but also promote early detection and community engagement in underserved areas.

Through strategic capacity building and systems support, HSDF is demonstrating how local PHCs can be transformed into resilient hubs for chronic disease prevention and care contributing to stronger, more equitable primary healthcare in Nigeria.
