HSDF applied its expertise in health systems research, policy analysis, and implementation science to explore factors that influence successful programme transitions. The project focused on:

• Examining approaches to locally led programme transition and sustainability.
• Identifying factors that support effective government ownership.
• Analysing institutional and system-level requirements for successful transitions.
• Generating evidence to inform policy and programme design.
• Documenting lessons and insights from transition experiences.
• Contributing to broader discussions on sustainable health systems strengthening.
